From Jawbone Forums
This is what worked for me:
Reset the iPhone. Start with the phone powered ON and then PRESS + HOLD both the HOME (on front of phone) and SLEEP (on top of phone) buttons at the same time until the screen flashes blank.
For the ICON: Starting with the Jawbone OFF, press and hold the TALK button (top of device). As you hold the TALK button, power the headset ON and continue to hold TALK button.
Please note: if you’re pairing with an iPhone and you get a message that says “Jawbone Not Paired” or “Headset Not Paired” please press on that message. This will prompt the iPhone to continue with the pairing/connection. The message will say Connected not Paired.

Shouldn’t need to restart the iPhone. Just make sure Bluetooth is set to On for the iPhone then follow the instructions for the ICON (hold down Talk button then start headset). Worked like a charm.

How to pair iPhone 4 with Jawbone Icon?
1. make sure bluetooth is off on phone and jawbone icon power is off.
2. press and hold jawbone talk button
3. turn jawbone on while still holding the talk button
4. turn iPhone bluetooth on now
It’s that simple on iPhone 4 with iOS 5.

Thank you so much! I thought I was loosing my mind. I had my iPhone “forget” my Jawbone ear piece. When I wanted to pair it again – it wouldn’t. All I had to do was reset my iPhone 4s by turning it off, and then back on again – your instructions saved me a trip to the Apple Store.

I had numerous little problems getting my iPhone activated and learning its features. But I had never had problems on my previous phone with the Jawbone Icon. Suddenly nothing worked; the instructions for both devices made it seem so easy.
